If it makes you feel better... back in '04 I was working for a 'large' bookmaker (betting company) and I was on the late shift.. To cut a long story short.. there were 2 apps open in a window on a server.. and I was tired and shut the wrong one down (was about 11/11:30 at night). This meant various tasks didn't run..and the ones that did run, failed.

Jump to the next morning, none of the betting shops can use their EPOS tills and had to revert back to paper to take bets whilst the cock up was sorted (no failsafes had been implemented).

They lost about half a days trading, estimated cost 3.5-4 million... all from 1 little click of the wrong button...
