Wedding favour please

Hello,
Wondering if someone could do me a favour as I can't find any in the shops!
Looking for someone to create what will be essentially a card probably 4cm height by 8cm wide saying something like "Please choose the one song that will get you up and bopping away" or similar and leaving them space to write that one song, dotted lines or something! Our theme colours are midnight blue and ivory so if you wanted to incorporate those that would be cool but happy with any others if they'd look better, and some wedding related or music type things on them.

Will use this template to copy into Word and print off little cards to include in our invitations to get people to fill them in and return to us when they rsvp!

If anything doesn't make sense let me know

A small card design to send out with the wedding invites that people can request a track they would dance to at the reception.
They would fill this card in with the info and return it with the confirmation that they will attend the wedding.

Xploit, could I be a pain and ask for the card to be on a transparent background? Jon's just tried to print them and said that would probably be better! Is that okay?
